DSME - Information for Patients

What is Diabetes Self Management Education (DSME)?

DSME is a program designed to teach you every day skills necessary to manage your diabetes. The University of Texas Health Services (UTHS) DSME educator will cover popular topics such as nutrition and reducing risks, while also addressing other less talked about issues such as coping strategies and problem solving. Whether you are new to diabetes, or have had it for years, knowledge is power in the battle for blood sugar. The UTHS DSME educator divides the program into 3 sessions covering the following topics: monitoring, reducing risks, problem solving, healthy eating, increasing activity, medications, and coping strategies. Although each class is important, you can start the program at any point since new information is presented at every session.

What can I expect?

Each class is approximately 3 hours in length with a relaxed, engaging and interactive format. You will be given a binder in which to keep class handouts and notes. During class, we encourage you to ask questions related to  your biggest obstacles to achieving success. Classes will provide an excellent support network and  the UTHS DMSE staff ask you to come prepared to work on taking control of your diabetes.

At the end of each class, the educator will ask you to complete a “SMART” goals list. Please take a moment to review this form and begin to identify areas where you believe some improvement can be made. Knowing where you want to focus will help you to develop your own personalized goals as you move through the classes.

How do I get started?

In order to get started in the program, we will need to obtain a referral from your provider. Simply provide us with your provider’s contact information, and we will be happy to obtain the referral on your behalf. If you do not have a provider, one of the providers at UT Health Services will meet with you and complete your referral form. You can find the Referral Form in the link at the bottom of the page.

Next, review the schedule of classes to determine which time, day and location is best for you. A proposed class schedule is available via the link at the bottom of the page. Classes are being added, so check this site regularly. Our scheduler will contact you when we receive your referral. We want to reserve your spot as soon as possible as class size is limited.
